By scheduling appointments with Empowered Healing, you are agreeing to attend these appointments and engage in treatment. If you wish to revoke this at any time, 24 hours notice is expected. If less than 24 hours notice is provided, a late cancellation/reschedule fee of $150 may be charged. After three late canceled or no show appointments, the patient-provider relationship may be terminated. If requested, alternative providers will be suggested, but it is the patient’s responsibility to determine financial eligibility and schedule. Upon termination of services, a plan will be discussed, including 90 days' worth of medications when appropriate (as deemed by PMHNP). Exceptions to this policy are made at the discretion of the practice.
